Our consultation form collects your name, email address, phone number, coverage interest, preferred language, optional note, and the contact and SMS permissions you choose. We record the submission time, source page, disclosure version, and separate SMS consent choices. Do not submit medical information, Social Security numbers, policy documents, payment details, or financial account numbers through this form.
How we use information
We use your details to respond to your request, discuss insurance services, coordinate appointments, and provide customer care. Informational SMS may include inquiry responses, inquiry follow-ups, customer care, and appointment confirmations and reminders. Promotional SMS about our insurance services and coverage options are sent only with a separate marketing SMS opt-in. Providing a phone number or requesting a quote alone does not authorize SMS.
Mobile information and SMS consent
We do not disclose mobile information to third parties or affiliates for their marketing or promotional activities. We do not sell or rent mobile numbers. Vendors helping us deliver messages or provide customer support may process information only to perform those services for us. SMS opt-in records and consent are not shared with third parties or affiliates for their own purposes, including marketing.
Service providers
We use GoHighLevel to manage consultation requests and consent records, and Cloudflare to host the website. These providers process information as needed to operate the site and support our communications. This use does not grant another business permission to market to you. We may disclose information when legally required or necessary to protect the security of our services.
Website measurement
The site stores campaign parameters in your browser to attribute consultation requests. When enabled, Meta Pixel or Google Analytics may measure visits and conversions. We do not include your form answers, email, phone number, or note in advertising event parameters. Your browser settings can restrict local storage and tracking.
Your choices and requests
You may choose informational SMS, marketing SMS, both, or neither. Neither consent is a condition of a quote, purchase, or service. Reply STOP to a message to unsubscribe from our SMS program; a final opt-out confirmation may be sent. Reply HELP for assistance, or use the support email below. Contact us to request access, correction, or deletion of your information, subject to applicable recordkeeping obligations.
Retention and security
We keep information for as long as needed to respond to your request, provide services, document consent, and meet applicable obligations. Access is limited to the people and providers who need it for those purposes. No website or communications system can guarantee absolute security.
Policy updates
We may update this policy when our practices change. The date above identifies this version. A policy change does not by itself grant us a new SMS permission.
